Requesting Medical Records

To request a copy of medical records from care received at St. Francis Hospital & Health Centers, please complete an Authorization for Release of Confidential Information and return it to:

What you should know

This process is in compliance with Federal HIPAA guidelines. Please allow up to 15 business days for your records to be processed and mailed to the specified address.

St. Francis contracts with Smart Document Solutions, LLC, for this service. There is a fee for processing your request.  You will receive an invoice along with the records. 

Medical records are destroyed after 10 years.

Information on birth certificates are processed by the state as well as the county health department. 

The HIM department processes release of information from 8 a.m. to 4:30 p.m. Monday through Friday.

Who is authorized to sign for release of my medical records?

  1. You, the patient
  2. Your Healthcare Representative or Healthcare Power of Attorney if you are unable to sign. Legal documents must be provided.
  3. If you are under 18, your parent/parents may sign for you. 
  4. Legal guardian if appropriate.  Documentation must be provided. 
  5. For deceased patients, the Executor of the Estate is the first person who can request copies of records.  Death Certificate and Estate documents must be presented.
